By making this purchase to the campaign of Peggy DuCharme, Candidate for Councillor, Township of Ashfield-Colborne-Wawanosh, I declare that:

• I am an individual who is normally a resident in Ontario.

• I am making this contribution using my own funds and not exceeding $1,200.

• This contribution is not being made on behalf of another person, a corporation, or a trade union.

• I understand that corporations and trade unions are not permitted to contribute to Ontario municipal election campaigns.

• I understand that my contribution is subject to the contribution limits established under the Municipal Elections Act, 1996.

• The information I have provided is true and complete, and I understand that my contribution and identifying information will be recorded as required by Ontario election law.
